This is not the form or department for Food Stamps. You will need to contact the Department of Health and Human Services (See Links) | | You must fill out this form and submit it weekly to the Welfare office. To qualify for continued assistance you must apply for jobs at four employers and they must sign the form. | | You must bring this form to the NH Employment Security office and have them sign it weekly attesting that you have participated in programming or job search. |
